When you register as a patient, we will ask you to show us some form of identification (ideally a photo), and proof of where you live. For this reason, the surgery will not accept postal applications for registration. If you are a family and wish to register, at least one member of the family must be present when registration is completed. The receptionist will provide you with a GMS1 form and a practice booklet that contains a New Patient Assessment Questionnaire. Completing this questionnaire helps us to identify your medical needs quickly. A GMS1 and Questionnaire must be completed for each individual applicant.

Please bring identification with you when you register.

When you register with the Practice, you will be registered with a specific Doctor and this will be recorded in your medical records. Please ask one of our reception team if you wish to know the name of this; your accountable GP. This GP is responsible for your overall care at the Practice. If you have a preference for a particular GP, please let us know (in writing) and we will try to accommodate your request.
