Requests for repeat medications not pre-printed on re-ordering slips
We receive a number of requests for repeat medications other than those pre-printed on repeat slips. These are usually for medications which a patient needs to reorder infrequently, or for items that a clinician has decided needs to be prescribed as an acute item for medical reasons.
We are happy to receive such requests via e-mail/ "the waiting room", written on the original repeat slip when re-ordering your other medications or written on our repeat prescription request form which you can download here. The quality and safety of the service we provide our patients is our prime concern and to enable this we need to know in all cases:
- Name of requestor
- Signature of requestor
- Date of request
- Contact number of requestor
- Whether the requestor is the patient or not.
- If not the patient we also need to know:
- Patient name and Date of birth
- Your relationship to the patient.
- Name and amount of medication
- The reason for the request.
We have started to print prompts for this information at the bottom of repeat ordering slips and our reception team will be reminding patients and their representatives of our requirements. We will be unable to accept requests for medications which not pre-printed on re-ordering slips without the above information.
Please allow 72 hours (excluding Bank Holidays and weekends) for us to process your repeat prescription request.
If you have any problems, please telephone the surgery, but note that urgent requests for repeat medications do not generally require an appointment with a GP or AP.
